With Meds-check service we provide an in-pharmacy review of your medicines, focusing on education and self-management. We aim to:

  • Identify problems that you may be experiencing with your medicines;
  • Help you learn more about your medicines including how medicines affect medical conditions;
  • Improve the effective use of your medicines; and
  • Advise how to best use and store your medicines.

You’ll leave your appointment with enhanced understanding about your condition, the prescription medicines and non-prescription medicines or vitamins you’re taking, and any medicine interactions. You’ll know when and how to take your medicines to maximize benefits and minimise side effects. And you’ll take away a handy wallet-sized card listing all your medicines – a valuable source of information on prescription medicines when you visit the doctor, dentist or the hospital.

This service is funded as part of the Fifth Community Pharmacy Agreement for Eligible Consumers. To be eligible for government funding of a MedsCheck you must:

  • Be a Medicare and/or DVA cardholder;
  • Have not received a MedsCheck, Diabetes MedsCheck, Home Medicines Review (HMR) or Residential Medication Management Review (RMMR) in the last 12 months;
  • Be living at home in a community setting;
  • Be taking five or more prescription medicines or have had a recent significant medical event. *A significant medical event can be defined as a recent event or new diagnosis that has the potential to impact on the consumer’s medication adherence or knowledge of their medicine regime and may increase the risk of medication misadventure.

Before you come for the appointment:

  • Check you eligibility for government payment for the Meds-check
  • Bring all your current medicines (including prescription, non-prescription, vitamins, minerals, herbals, nutritional supplements) and any medicine/monitoring device to the appointment

During the appointment

Our pharmacist will sit down with you in a private area and give you a Consent Form to sign. Then gather relevant information from you or your carer including:

  • A complete list of your prescription, non-prescription and complementary medicines (Medicines List)
  • any allergies you may have
  • any chronic medical conditions

Review and discuss the use of all medicines and medicine monitoring devices including:

  • How well you adhere to your medicine regimen
  • What information you need to help you better understand and use your medicines
  • Provide you with education and guidance on the correct use of your medicines and/or monitoring device
  • Discuss management of chronic condition(s) including lifestyle factors related to medicines use and self-management.
  • Attempt to resolve any medicine-related issues that have been identified from the information available.

Develop a written Action Plan including goals and actions agreed with you and any follow-up with your GP and/or other healthcare provider(s). Provide you with a Report that includes a Patient Medication Profile (Medicines List) and an Action Plan.
